Composition

 

Each tablet contains :

Metformin HCl   500 mg , 500 mg.

Glibenclamide   2.5 mg  ,    5 mg.

 

 

 

 Indications

 

Description :

- is a fixed combination of two oral Sulfonylurea (glibenclamide) and Biguanid (metformin) anti-diabetic agents.

- This combination has a complementary mechanisms of action to improve glycemic control, glibenclamide stimulates the release of insulin from the pancreas, metformin decreases hepatic glucose production, decreases intestinal absorption of glucose and improve insulin sensitivity by increasing peripheral glucose uptake and utilization.

 Indications :

-   is indicated as:

* an adjunct to list diabetic diet and balance exercise to lower blood glucose in patients with
adult-onset diabetes mellitus type II.

 

 

Dosage & Administration

 

- is  taken  one tablet per day, the dose may be increased to 3-4 tablets divided dose/day as recommended by the physician, as following :

* 1 intake per day at breakfast if the dosage is 1 tablet daily.

* 2 intake per day at breakfast and at dinner if the dosage is 2 or 4 tablets daily.

* 3 intake per day at breakfast, at lunch and at dinner if the dosage 3 tablets daily. 

Contraindications :

-   is contraindicated in :

* Hypersensitivity to sulfonylurea or biguanid.

* Insulin dependent type I.

* Loss of diabetes control with ketoacidosis.

* Hypoglycemia due to not eating carbohydrate enough, prolong exercise or certain adrenal and pituitary disorders.

 Precautions :

is used with caution in :

* Renal and hepatic impairment.

* Lactic acidosis.

* Heart failure or recent myocardial infarction.

* Severe infection (respiratory tract infection & urinary tract infection).

* After radiological examine with iodinated contrast material for 2 days.

Drug interactions :

- effects is changed if concomitantly administered with β. blockers, estrogens,
corticosteroids, MAOI, calcium channel blockers, diuretics, oral contraceptives, miconazole.

Side effect :

-  has side effects like :

* Hypoglycemia (tremor, blurred vision, sweating, palpitation).

* Rarely nausea, vomiting, diarrhea.

* Skin reactions (urticaria, itching).

 Instructions to the patients :

- Swallow the tablets as whole with a drink of water.

- Take the tablets with food at regular interval.

- Tell your doctor about other medicines you are taking including non prescription drugs.

- Tell your doctor about any other diseases you have and the side effects appear from                   .

- If you forget to take the next dose at the usual time don't double the dose.

- Visit your doctor for regular checks, learn how to monitor blood or urine sugars and urine
ketones regularly , don't skip meals. If you are exercising much more than usual you may need extra snacks to avoid hypoglycemic side effects. If you have mild symptoms of low blood sugar, eat or drink sugars, it is important for you and your family to recognize of hypo & hyperglycemia to reach your glycemic control and to avoid their serious side effects.

 Use in pregnancy and lactation.

-  not recommended for use in pregnancy and lactation woman.
